Do Traditional Braces Hurt? What Tempe Patients Should Know

Traditional braces generally do not cause sharp pain, but patients should expect mild to moderate discomfort, soreness, or a dull ache as teeth begin to shift. This discomfort is temporary, most commonly occurring immediately after the initial placement and after routine tightening adjustments.

Most Tempe patients find traditional metal braces far more comfortable than expected, and the experience settles into routine within a few days. Mild pressure and tenderness for 3 to 7 days after placement is the typical pattern, with even shorter periods after each adjustment, consistent with American Association of Orthodontists patient guidance.

At Smilebliss Orthodontics, Board Certified Orthodontist Dr. Hojae “Jae” Choi walks every patient through what to expect, and most leave their consultation feeling at ease.

Does Putting Braces On Hurt?

No. The placement appointment is comfortable and painless. The orthodontist bonds brackets to your teeth and threads the first archwire through them without drilling, injections, or numbing. The appointment usually takes 60 to 90 minutes. Light soreness arrives 4 to 12 hours later, once the archwire begins guiding teeth into their new positions.

How Long Does Soreness Last?

Soreness from the first placement lasts 3 to 7 days and peaks around day two, then fades quickly as the mouth adjusts. Most patients return to work or school the next day, just sticking to softer foods for the first week. After each adjustment appointment (every 4 to 8 weeks during treatment), expect a brief, milder soreness lasting 1 to 3 days.

What Does It Actually Feel Like?

Braces cause two distinct sensations:

  • Pressure and tenderness on the teeth. A dull ache, especially when biting or chewing. This is the orthodontic force gently doing its job.
  • Cheek and lip tenderness from the brackets. Brackets are smooth but new to your mouth, and the inner cheek needs a week or two to adapt. Orthodontic wax handles most of it.

Sharp pain is uncommon and usually points to a simple fix: a loose bracket, a wire that needs trimming, or a bonded bracket that shifted slightly out of place.

How Do You Stay Comfortable in the First Week?

Comfort tips that work in the first week:

Applying orthodontic wax on the dental braces. Brackets on the teeth after whitening. Self-ligating brackets with metal ties and gray elastics or rubber bands for perfect smile
  • OTC pain relievers (ibuprofen or acetaminophen) taken before the soreness peaks, not after.
  • Cold water and cold foods. Ice water, yogurt, smoothies, and popsicles ease the tenderness.
  • Soft foods for 3 to 5 days. Pasta, eggs, soup, mashed potatoes, soft bread.
  • Orthodontic wax on any bracket that irritates your cheek.
  • Warm salt water rinses to soothe the gums.

Avoid sticky candy, hard nuts, ice chewing, and crunchy raw vegetables throughout treatment to protect the brackets, especially in the first few days while teeth are settling in.

Are Braces Different for Adults and Teens?

Adults and teens have very similar experiences with braces. Adult bone is denser, so teeth move slightly slower, and treatment can run longer overall. Adults sometimes notice a bit more cheek tenderness in the first few weeks. Both groups adjust within a couple of weeks.

When Should You Call the Orthodontist?

Most soreness is normal and resolves on its own. Call your orthodontist if any of the following happens:

  • A bracket pops off or shifts
  • A wire end pokes the cheek
  •  Severe discomfort that does not respond to OTC pain relievers
  • Soreness lasting longer than 7 days after placement or 3 days after adjustment
  • Visible swelling around the gums

While waiting for the appointment, cover any wire end with orthodontic wax and stick to soft foods to keep things comfortable.
